Initial Visit

On your initial visit, please bring the following items:

• Your insurance card
• A referral from your physician (if your insurance plan requires it)
• Personal identification
• List of allergies
• List of current medications (prescription or over-the-counter) including name, dosage, and frequency
• Any related X-rays
• Records from the referring physician

Dr. Chang is an attending surgeon at the following hospitals (in alphabetical order):

• Hackensack University Medical Center, Hackensack, NJ (application pending)
• St. Joseph's Hospital and Medical Center, Paterson, NJ
• St. Joseph's Wayne Hospital, Wayne, NJ
• The Valley Hospital, Ridgewood, NJ

He also offers outpatient surgical services at ambulatory surgery centers in Englewood Cliffs, NJ, and Fair Lawn, NJ.

Payment and Insurance

David T. Chang, M.D., F.A.C.S., is a provider for Medicare and most major insurance plans. We provide insurance billing. Please note that your insurance company may require a co-payment at the time of your appointment and that any costs that are not covered by your insurance are your responsibility. Our staff will work closely with patients who require a payment plan.
